Insurance and Financial Policies

For patients with no health insurance coverage, we do offer time of service and financial hardship rates.  Please call the front desk at Apex Wellness Center (503-775-6885) for details.

We require payment in full (including co-pays, co-insurance and deductibles) for all services rendered and medicinary items purchased at time of service.

Current accepted forms of payment are cash, check or credit or debit cards

Insurance

We accept the following health insurance plans. If we are not in network with your specific plan, we can submit claims on your behalf as an out of network provider.

• Kaiser (Heraya Health) (Eleanor Jones is currently accepting new Kaiser/Heraya Health patients. Mary Jean Brinkman is not accepting new Kaiser/Heraya Health patients. )

MASSAGE

• Regence – Blue Cross Blue Shield (Bryan and Phoenix are in network. Raegan is currently in the credentialing process.)
• Kaiser/Heraya Health (Bryan Warren and Raegan Lancaster)
  • Phoenix will not be in network with this plan

Please contact us with your insurance information by phone or email and include the subscriber’s name, subscriber’s date of birth, insurance ID number, and group number. If the subscriber is different from you, please also provide your name and date of birth. This allows us to provide a courtesy verification of your benefits prior to your appointment.

Please note that this verification is only an estimate of benefits and is not a guarantee of payment.

Cancellations and No Shows

When you schedule an appointment we happily reserve that time especially for you. Please kindly give us 48 hours notice to cancel an appointment. If you do not provide us with at least 24-hour notice of your intent to cancel an appointment, you will be charged a late cancellation fee of $75.
